Do You Fit Any of These Characteristics?

—Are you 18 years of age or older with mental, behavioral, and/or emotional disturbances, which have resulted in significant impairment?

__Do you require help in basic living skills, such as maintaining personal hygiene and nutrition, or preparing food and managing finances?

—Do you have a documented history of behavioral issues or mental health issues that substantiate the need for mental health support services?

—Do you have targeted behavior issues demonstrating an inability for you to function within the home, work place or community as evidenced by the risk of removal from one or all of these areas?

—Do you have difficulty in the cognitive ability to recognize personal danger or inappropriate social behavior?
